Paediatric Physiotherapy in SDM College of Medical Sciences & Hospital

A talk on the achievements of Paediatric Physiotherapy over the past decade. The department started as part of the general physiotherapy department in 2007 with basic materials and instrumentation. The total number of treatment sessions was approximately 500-600 sessions then. Today we have an independent 2000 sq. ft. area, with 4 times the instrumentation. Today we have provision for sensory integration therapy, visual stimulation and computerized hand rehabilitation. We treat an array of conditions including cerebral palsy, meningomyelocoele, bowel and bladder incontinence, syndromes, brachial plexus injury, oromotor problems, etc. Also the average age of reference for therapy has come down to 6 months as compared to 3 years before. We plan to expand our department and venture into virtual reality and gait laboratory in future. Today we cater to children not from all over North Karnataka but also other parts of Southern and Western India, children from Saudi Arabia, Sharjah, Canada and Egypt. We want to see the department as the best when compared to any center anywhere in the world.

Lasers have a wide variety of clinical applications in Dermatology.
 
Three LASERS are currently being used in the Department of Dermatology, SDMCMSH.
 
Diode LASER for removal of unwanted hair which results in permanent hair reduction. Useful in conditions such as hirsutism, PCOS, faun tail and hypertrichosis.
 
Carbon dioxide (CO2) LASER for treatment of acne scars, chicken pox scars, burn scars, keloids, hypertrophic scars and post-surgical scars. The fractional mode in CO2 LASERS is safer and has a lesser downtime of recovery than the conventional mode.
 
Nd:YAG LASER for removal of pigmented conditions such as freckles, melisma, birthmarks and tattoo.

A Cross Sectional Study of Normal Variations of Sphenoidal Air Sinus Through CT Scan in North Karnataka

ABSTRACT
 
Introduction: In the humans, paranasal air sinuses play various roles like- make the skull light weight, add the resonance to the voice etc. We have frontal, ethmoid, maxillary and sphenoid sinuses. Out of these, sphenoid sinuses (SS) have got the importance because of trans-sphenoid approach for the pituitary surgeries and skull base surgeries. Therefore, the knowledge of anatomical variation of SS has a significant role in surgical access of pituitary fossa and also, can anticipate the difficulties in surgery.
 
Aim: This is the study to find out the variations of SS and their relations to adjacent neurovascular structures with surgical point of view.
 
Materials and Methods: Computed Tomography (CT) scans of 150 living subjects (53 females and 97 males) were taken in coronal, sagittal and axial planes. Contiguous slice CT technique was used with 1 mm section thickness. The p-value was calculated to find out the association between extent of pneumatization of sphenoid sinus and protrusion or dehiscence of maxillary nerve (MN), vidian nerve (VN), optic nerve (ON) or internal carotid artery (ICA) in the sinus. Chi square test was used for the calculation of p-value.
 
Results: It was observed that there were 2 cases with conchal pneumatization, 29 cases with pre-sellar pneumatization, 47 cases with sellar pneumatization, 72 cases with post-sellar pneumatization. It was observed that pneumatization of greater wing of sphenoid sinus (GWS) in 40 cases, pterygoid process (PP) in 39 cases and anterior clinoid process (ACP) in 34 cases. Onodi cell were noticed in 17 sinuses. Protrusion of ICA was found in 27 sinuses. The p-value was highly significant for the association between pneumatization of sphenoid sinus and protrusion or dehiscence of nerves or ICA in the sinus. Chi square test was used for the calculation of p-value. Posterior attachment of Inter Sinus Septa (ISS) was noticed in the midline in 36 individuals (24%) and deviated from midline in 113 individuals (75%). Accessory septae were noticed in 76 sinuses (51%).
 
Conclusion: Due to widespread pneumatization of SS, it lies in close relation with MN, ON, VN & ICA. Newer techniques were helpful for the safety of the patient during intra-operative and post-operative period.

Problem Based Learning, Need of Hour?

Abstract:
 
Introduction: Newer methods have to be introduced in teaching methodology in medical curriculum which is student centered. Problem based learning is one among them.  The present study was conducted to assess perception among 1st year M.B.B.B.S students on Problem based learning and to compare its effectiveness with conventional didactic lecture.
 
Methods: It is an interventional Study which includes 100 Ist year M.B.B.S. students. They were divided randomly into 2 groups and subdivided into 3 subgroups. Two different topics were selected for Problem based learning for both groups. PBL sessions were carried out for around 3 weeks. At the end of PBL sessions comment feedback were recorded using validated questionnaires using 5 point likert scale. At end of 3rd week Counter learning was done by conducting didactic lecture of different topic. At 6th week written test on both the topics was conducted. Means of the test scores were analyzed using un paired‘t’ test.
 
Results: More than 90% of learners felt that Problem based learning increased their understanding of the subject, critical thinking ability & encouraged self directed learning. When compared to didactic lecture mean of test scores is significantly higher in Problem based learning group. (5.94±1.37, 6.48±1.39,   p=0.05)
 
Conclusion: This study concludes that, PBL is a good teaching – learning tool for teaching physiology for first year M.B.B.S students.

Scope of Arthroscope in Orthopedics

Arthroscope is a component in the device used for doing Arthroscopy. Its used always with image processing camera and light source. Arthroscopy itself is derived from Scopien to see and Arthro meaning joint. It is similar to laparoscopy but its confined to Joints. Most commonly it is done on Knee, Shoulder, Ankle and Hip in the order of frequency. Many traumatic and pathologic conditions can be treated or diagnosed with this. Compared to open procedures it is advantageous in having less morbidity, less hospital stay, accurate surgery, less infection and hence early rehabilitation of the patient reducing the overall cost to the patient. Few of the pathologies that can be addressed with this are,, in knee, Meniscal repairs, menisectomies, ligament reconstructions, cartilage repair, loose body removal, retinacular release and fracture fixations. In shoulder Bankart’s repair, Rotator cuff repairs, Subacromial decompression, capsular release and biceps tendon issues. Though it has few complications like instrument breakage, infection their rate is much lower compared to open procedures. Hence orthopaedics, is gradually shifting from all open procedures to all closed procedures by using an arthroscope.
